Responsibilities

Assist patients with personal care and daily living activities, including bathing, dressing, and mobility support Support the implementation of individualised care plans in collaboration with healthcare professionals Maintain clear and effective communication with patients, families, and team members to ensure consistent care delivery Document observations and updates accurately using IT systems in accordance with organisational policies Provide companionship and emotional support to enhance patients' quality of life Assist with medication reminders and basic health monitoring as required Support the organisation of activities that promote patient wellbeing and engagement Ensure compliance with health and safety regulations at all times

Experience

Previous experience working in a care home or home care environment is advantageous but not essential; training will be provided for suitable candidates Good understanding of care plans and the ability to adapt support accordingly Proficiency in IT systems relevant to healthcare documentation and communication tools Strong communication skills in English, both verbal and written, to effectively interact with patients, families, and team members Valid driving licence is preferred to facilitate travel between different care locations Demonstrated ability to work compassionately, responsibly, and as part of a multidisciplinary team
