We are seeking a compassionate ,dedicated and Experienced Healthcare Assistant to join our team. In this role, you will provide essential support to patients in a care home or home care setting, ensuring their comfort and well-being. You will work closely with healthcare professionals to implement care plans and assist with daily living activities, fostering a supportive environment for our clients.
We are a CQC Outstanding rated Care Home with Gold Standard Framework accreditation in end of life care therefore your knowledge and experience of care need to be at a high level.
Duties
Assist patients with daily living activities such as bathing, dressing, and meal preparation.
Support the implementation of individual care plans tailored to each patient's needs.
Communicate effectively with patients, their families, and healthcare professionals to ensure continuity of care.
Maintain accurate records of patient progress and any changes in condition using IT systems.
Help with mobility assistance and transport patients as required, including driving if necessary.
Ensure the cleanliness and safety of the care environment, adhering to health and safety regulations.
Participate in training sessions and professional development opportunities to enhance skills and knowledge.
Experience
Previous experience in a nursing home healthcare setting is essential
Strong communication skills in English are required for effective interaction with patients and team members.
Familiarity and experience with digital care plans is beneficial.

Pay: 12.85-14.00 per hour
