DT is 20 years old and unable to communicate, immobile, incontinent, cognitively impaired and causes him to suffer from life threatening seizures and can have up to 20 cluster seizures in 24 hours. At present he lives with family in own home and attends school Monday - Friday. His medical diagnosis are Chromosome 15 disorder partial triplication of chromosome 15, Partial Triplication, Intractable Epilepsy, Reflux Gastroesophageal, Severe Oesophageal dysphagia, Connexion 26, Sensorineural hearing loss, Scoliosis, Minimal cerebellar volume loss, Knee contractures, Non-convulsive status, Spinal kyphoscoliosis, Hypotonia ,Severe developmental delay, Lennox - Gestaut syndrome ,Epilepsy, Non-verbal , Autism, Oesophageal reflux, Visual impairment, Sensory hearing loss and is wheelchair bound. Care staff to feed DT orally (Minced and moist food) prepared by family. His medication is given orally and via the PEG. Job description:

Responsibilities

Provide patient care and support to individuals in various settings, including senior care and assisted living facilities. Assist with daily living activities such as bathing, dressing, and meal preparation. Administer medication as prescribed and monitor health conditions Perform heavy lifting when necessary to assist clients with mobility or transfers. Maintain a clean and safe environment for clients by adhering to health and safety regulations. Collaborate with healthcare professionals to ensure comprehensive care plans are followed. Document care provided and report any changes in client condition to the appropriate personnel.

Service user Description

IService User is 29-year-old lady, diagnosed with rapid progression multiple sclerosis in 2014. Fully dependent on her parents and care workers for all activities of daily living, who requires Tier 3 Care Workers with at least 2 years of experience in supporting her with specific needs, including tracheostomy tube management, cough assist machine operation, NIPPY 4+ Ventilator assistance, Humidify processes, oral and deep suctioning techniques, oxygen therapy administration, PEG Feeding, medication administration via PEG, and infection control proficiency.
