Health Care Assistant -
All healthcare assistants are to provide dignified, compassionate and high quality care.
All staff to deliver care with a person centred approach.
Summary of Roles & Responsibilities
Assist our residents with personal hygiene needs as required and appropriate.
Assist our residents to maintain pressure relief and skin integrity as required and appropriate.
Assist residents with continence needs and complete assessments as required and appropriate.
Provide a safe and comfortable home environment for our residents.
Follow care plans, whilst promoting privacy, dignity and individualised care.
Promote and encourage our residents to make their own choices where possible and always act in the best interests of the individual.
Undertake training in areas of Safeguarding, Mental capacity, DOLS/LPS, Record keeping,
Fire training and any other training provided or that is mandatory to working within the home.
SAFEGUARDING IS EVERYONES RESPONSIBILITY
Key tasks
Assist individuals with personal care.
Assist individuals with diet and fluids and document accordingly.
Provide support and care
Provide good documentary evidence of care provided at point of care.
Recognise changes in needs and respond, report and document accordingly
To remain fully compliant with training competencies
Staff must effectively communicate and report any issues to nursing/senior staff members
Effectively communicate between all departments within the home. We are one team.
All staff to comply with PPE regulations within the home.
Report any defects with equipment or damage to the property.
Accountability
All staff are accountable and responsible for their own actions.
Personal care
Healthcare assistants to follow our residents individual care plans and provide excellent individual care.
Always consider an individuals preferences or requests.
Assisting an individual to wash-
When assisting a resident with personal care and assisting a resident to have a wash, staff to ensure they are washing and drying residents properly. Staff to ensure groins, armpits, under breasts are being washed and dried properly.
Nails
- ensure that nails are kept clean and to trim when necessary.
Oral hygiene-
ensure that we are assisting residents with good oral hygiene. Ensure dentures are cleaned and soaked overnight.
Eyes-
When cleaning a resident's eyes, use clean wipes for each eye to prevent any cross contamination. Report any concerns.
Hair-
ensure we are washing an individual's hair were necessary and brushing and styling appropriately.
Creams-
Staff to ensure prescribed creams are being used e.g. emollients, analgesic gels, fungal creams. If staff are unsure as to what the creams are for or were they are to be applied, please check with nursing staff or senior care team.
Bathing and showering-
staff to ensure we are providing our individuals to have bath or shower when necessary, appropriate or requested by an individual.
Assisting a resident to dress-
ensure we are assisting our residents to dress appropriately and involving them to choose clothes. Consider the environment and take this in to account when assisting individuals to dress. Ensure appropriate footwear is worn especially for residents who mobilise.
Aids-
Staff to ensure an individual has any aids they may need, e.g. hearing aids, check they are working and are placed in an individuals ears correctly. Glasses- ensure they are clean.
Catheter care-
Ensure catheter sites are cleaned. If a resident has a catheter, staff need to ensure they are using x2 leg straps when attaching securely to an individuals leg and ensuring the tubing is not kinked to prevent any bypassing or blocking of the tube. Drain legs bags when they are above 200mls to prevent it pulling on a resident. If an individual with a catheter is in bed ensure a night bag is attached and place on a stand and that tubing is not trapped or kinked as this can lead to the catheter bypassing.
Continence care-
Ensure we are assisting an individual with their individual continence care needs, assist an individual when required to use the toilet or assist them with continence care. Ensure prescribed continence aids are being used.
Regular pressure care-
Staff need to ensure individuals are being assisted with regular pressure relief and that individuals are not sitting in wheelchairs for long periods of times.
All staff to ensure they are reporting or communicating any concerns about an individual to nursing staff/senior care team. Document any concerns or issues.
Assisting a resident
Upon all interactions with a resident, staff should be diligent and consider:
An individuals appearance-
Staff to be diligent of an individuals appearance. For example,
Do they need assistance to wash their face or hands after meals? Do they need assistance to change their clothes after a meal. Depending on the environment, might they be too hot or cold, do they need their clothing adjusted.
An individuals environment
- Does the individual have things within their reach that they need or may want nearby for example, call bells to be within reach of an individual who uses the call bell system. If they have a table in their rooms, do they have a drink within reach, tissues, tv remote, books, magazines, snacks, their mobile phone, consider and ask what they may need or want. Do they want the tv or the radio on? Is the lighting ok in their room? Is the room temperature ok? If an individual is in their bed do they have adequate layers and bedding.
For residents that choose to sit in the lounge, are they sitting where they want to sit? Are they watching something on the tv that they have chosen to watch or if care staff? Might they require a blanket?
Document any interactions with our individuals, what you see, what you do and what you may talk about.
PPE
All staff must be compliant with the homes PPE policy.
All staff to ensure that they are donning and doffing PPE as per guidancHandwashing must be undertaken after each interaction with an individual
Staff to ensure face masks are worn at all times throughout the home.
Documentation
ALL DOCUMENTATION TO BE DONE AT POINT OF CARE
Staff to ensure all documentation is in the correct area on CMS.
Vitals- Fluid input/output-
document individuals fluid intake. Document fluid output when assisting an induvial with catheter care.
Weights-
record weekly and monthly weights.
Pressure area care-
document here after assisting an individual with any repositioning, pressure relief.
Continence-
Bowels and urine output, document the continence care that has been given.
Morning routine-
include all care that has been given. Document anything that an individual may decline. Elaborate on the care you have provided and your interaction with the individual.
Evening routine-
include all care that has been given. Document anything that an individual may decline. Elaborate on the care you have provided and your interaction with the individual.
Food and Drink-
If you are assisting a resident with their meals or drinks, document what the individual has eaten or drank. If you remove plates from a residents bedroom, record and document what they have eaten.
Night checks-
This can be used for visual checks throughout the night but continence care and pressure relief must be documented under the correct assessment.
ABC Charts-
Document any type of behaviour that is deemed challenging or inappropriate from any individual towards a staff member or another individual who lives within our home.
Visual checks-
Staff to ensure that Visual checks rounds are performed and documented for
ALL of our individuals. Elaborate for example, what is the resident doing, where are they, how do they seem, do they need anything?
Body maps-
Document any new bruises, sores, grazes, cuts, pressure sores, rashes. Any changes to a residents physical appearance must be documented and photographs taken and uploaded to CMS. Staff to ensure they communicate any issues with nursing/senior care team.
Document any interactions with our individuals, what you see, what you do and what you may talk about.
All documentation must be comprehensive.
IF IT ISNT DOCUMENTED, IT DIDN'T HAPPEN
All staff are accountable for their own documentation. Do not expect or assume that another team member has documented.
The home and the environment
All staff to ensure we are treating individuals rooms and possessions with respect. Staff to ensure we are leaving residents rooms clean and tidy and putting things back in the correct places. All staff to ensure they are respecting our individuals home.
Staff to ensure we are keeping corridors, communal areas, bathrooms, hallways, clean and tidy.
Hoists to be placed appropriately in the corridors.
Fire exits to be kept clear in the event of an emergency.
Staff to ensure that any maintenance issues are documented in the maintenance log so these issues can be addressed.
Doors are not to be propped open. If any door guards are not working, report and document in maintenance log.
Moving and Handling
All staff to undertake moving and handling training from a member of the senior care team.
Mobility aids-
Staff to ensure we are using the appropriate mobility aids for each resident as stated in their individual care plans. If an individual mobilises, staff to ensure appropriate footwear is worn by the individual. Any individual who uses a mobility aid must have the mobility aid within reach for them to use.
Hoists-
Staff must follow an individuals moving and handling care plan in regards to the correct hoist that should be used and the appropriate sling to be used.
Slide sheets-
Slide sheets must be used for any repositioning of an individual in their beds.
Sensor mats-
All staff to ensure that sensor mats are correctly connected to mounted call bell system and that they are working. Staff to ensure they are placed appropriately and do not present a risk or tripping hazard to the individual. If a sensor mat alarm is activated, staff must attend promptly.
Staff are to risk assess before undertaking any moving and handling.
All staff members have a responsibility to report any damage or issues with any moving and handling equipment.
