The Nautilus Welfare Fund is a UK-registered charity administered by Nautilus International, offering a range of specialist services to support retired seafarers and their dependants. Mariners' Park is based in, Wallasey on the Wirral Peninsular overlooking the River Mersey comprising a Care Home, Independent Living and Additional Support.
Job Purpose:
To work as part of a team to deliver a high quality, personalised care and support service to residents at the Mariners Park Care Home, helping people maintain their independence, health and wellbeing.
Key Accountabilities:
? Make referrals and advocate on behalf of the residents and act as a co-ordinator of care and support services provided by other agencies.
? Provide a programme of supervision and Personal Development sessions to level 0 & 2 staff.
? Undertake a number of checks regulatory checks and audits as directed by the Nurse or SCO.
? Encourage and promote healthily living, social and community activities for residents.
? Work as part of a team and participate in the delivery of service objectives as described in the Nautilus Welfare Service Strategic and Departmental Plan.
? Support the Registered Nurses in the delivery of their functional roles.
? Follow an assessment and plan to deliver personalised care, which may be in the form of bathing, washing, dressing, moving and handling, skin care, shaving, medication prompts/assistance and end of life care. Assisting at meal times as required. Additionally to undertake this with an activity focused outlook rather than TASK focused.
? Help residents that may have an agreed personal risk assessment, to stay safe but to be equally respectful of the resident's choice, control and right to take informed risks. Identifying any concerns and giving assistance where appropriate.
? Act in a caring, sympathetic and tactful manner when dealing with residents and relatives.
? Monitor the health and wellbeing of residents, responding as appropriate to any changes.
? Promote activities and spend quiet periods of the shifts facilitating resident activity whenever possible
? Record and report relevant information, relaying any concerns regarding the care of the residents to the nurse in charge.
? During working hours you will be responsible to emergency calls, ensuring appropriate action is taken
? Support residents to make comments or complaints about service or external services.
? Contribute and participate in staff meetings and staff training.
? It is essential that at all times, you will take care of and consider the appearance of the care home.
Specific Role (NVQ/QCF Diploma Level 3)
? Support Registered Nurses and SCO's in carrying out their 'link' roles
? Undertake routine clinical observations under the direction and/or supervision for Registered Nurses.
? Support Registered Nurses by undertaking some responsibility for assessment and key support to residents requiring "non-nursing care" as direct by the Registered Nurse. This includes supporting residential residents with medication and in some cases administering medication.
? Responsibility as and when directed by the RGN, Deputy or Manager to admit/discharge residents of the care home.
? Assessment of and Care Planning/Review for residents requiring residential support
? Supervise a group of staff and undertake supervision, PDPR, disciplinary, monitor sickness, competency and conduct and perform back to work interviews.
Typical Skills and Experience
? Holistic and person centred approach to supporting older people with experience of care and support planning and review.
? Willingness to work as part of a team and work on own initiative with the ability to develop and motivate others to continually improve performance.
? Willingness to undertake a comprehensive care and support induction and further planned learning and development which takes account of the recognised standards within the health and social care sector.
? A committed approach to maximising people's choice, control and inclusion and
safeguarding their human rights. With an understanding of diversity, beliefs and values.
? Excellent written and oral communication and good general IT skills in Microsoft Office
? Display a flexible attitude to towards work patterns/rotas (working anytime between 7am and 8pm, with weekend cover on a rota basis)
? Ability to carry out moving and handling of residents.
? An understanding of Care Quality Commission CQC and service delivery under this.
? Some knowledge of the current health and social care agenda as it affects older
people and the delivery of services.
General
You have a responsibility for ensuring that you are aware of the health & safety policies and procedures for NWF and that you take responsibility for your own health and safety whilst at work. Should you have any queries or questions you must raise these with your manager.
As we work with vulnerable adults, and come into contact with children, it is critical that you are able to recognise and respond to signs of or actual abuse and be aware of your responsibilities in relation to safeguarding. Training will be provided to support you in this area.
Mobile Phones are forbidden on the floor when caring for residents but can be used when on allocated breaks.

You have a joint responsibility with NWF to work with your line manager to ensure that you are adequately skilled to perform your job and participate in the annual learning and development programme.
It is essential that you are aware of the policies and procedures which support the work of NWF and Nautilus International as they apply to you, and you are expected to contribute to their ongoing review. If there are any policies and procedures you do not understand, you must notify your line manager immediately.
