Duties

Assist patients with personal care tasks, including bathing, dressing, and grooming. Support individuals with mobility and physical activities as needed. Implement and follow care plans tailored to each patient's needs. Monitor and record patients' health status, reporting any changes to the healthcare team. Provide companionship and emotional support to enhance the quality of life for patients. Maintain a clean and safe environment for patients by adhering to health and safety protocols. Utilise IT systems for documentation and communication purposes effectively. Drive to various locations as required to provide home care services.

Experience

Previous experience in a care home or home care setting is preferred but not essential. Familiarity with care plans and the ability to follow them accurately is advantageous. Proficiency in English is required for effective communication with patients and team members. Strong IT skills are beneficial for maintaining records and using healthcare software. Excellent communication skills are essential for interacting with patients, families, and colleagues.
