Job Overview


We are seeking a compassionate and dedicated Support Worker to join our team. The ideal candidate will have a strong background in providing care and support to individuals in various settings, including home care services. This role involves assisting clients with daily activities, ensuring their comfort and well-being while promoting independence.

Responsibilities



Provide personal care and support to clients, including assistance with bathing, dressing, and grooming. Administer medication as prescribed and ensure adherence to care plans. Assist with meal preparation and dietary needs, ensuring clients receive proper nutrition. Support clients with mobility and heavy lifting as required. Engage clients in meaningful activities to promote social interaction and mental stimulation. Implement behaviour management strategies as needed to support clients with dementia or Alzheimer's care. Maintain accurate records of client progress and communicate effectively with healthcare professionals regarding any changes in condition. Drive clients to appointments or social activities as necessary. Collaborate with case management teams to develop and update individual care plans tailored to each client's needs.

Requirements



Previous experience in senior care, home care, or hospice care is highly desirable. Knowledge of medical terminology and experience in patient care is essential. First aid certification is preferred; training may be provided for the right candidate. Familiarity with caregiving practices for individuals with dementia or Alzheimer's is an advantage. Strong communication skills and the ability to work effectively within a team environment. A valid driving licence is required for transporting clients when necessary. Ability to perform heavy lifting tasks safely and efficiently. A compassionate approach towards supporting individuals in need of assistance.
