Health Care Support Worker - Learning Disabilities (Primary Care)

We are seeking a compassionate and motivated Health Care Support Worker (HCSW) to join our dynamic Learning Disability Primary Care team. You will support registered nurses in delivering high-quality, evidence-based care to individuals with a learning disability across GP practices, nurse-led clinics, and health promotion initiatives.
Key Responsibilities:
Assist in improving access to healthcare and reducing health inequalities.
Support the delivery and quality of annual health checks.
Promote and implement reasonable adjustments in primary care settings.
Work within defined clinical competencies and protocols under nurse supervision.
Requirements:
NVQ/QCF/CQFW Level 3 in Health or Social Care.
Experience working with individuals with learning disabilities is desirable.
Strong communication, IT, and documentation skills.
Commitment to patient safety and confidentiality.

Support Identification & Record-Keeping :
Assist in identifying individuals with a learning disability and maintaining accurate LD registers.
Health Screening & Pre-Check Support :
Gather relevant health information and conduct screening under indirect supervision of a registered nurse.
Clinical Support :
Aid in assessing and managing patients' healthcare needs in primary care settings, including direct care tasks like phlebotomy, ECGs, and observations.
Reasonable Adjustments :
Help primary care services implement adjustments to improve accessibility for individuals with LD.
Training & Education :
Support delivery of training to primary care staff, carers, and individuals with LD on healthcare needs.
Accessible Information :
Contribute to the development and sharing of easy-read health materials.
Monitoring & Reporting :
Recognise normal clinical parameters and report abnormalities to the registered nurse.
Documentation & IT :
Maintain accurate written and electronic records in line with ABUHB policies.
Health Promotion :
Encourage self-care and illness prevention, respecting patient preferences.
Teamwork & Communication :
Work collaboratively across multiple primary care sites, maintaining professional relationships with patients and colleagues.
