You will be a core member of the Urgent Care West Kent team based at Unit G, Maidstone where you will be delivering care, both clinical and non-clinical to patients/ clients.
You'll also be key in supporting patients with their own self-care and independence. This service is part of the Urgent Care Services which provides a two-hour response to patients in crisis at home.
Our aim is to prevent unnecessary hospital admission and facilitate early discharge from the acute hospitals. We also provide unscheduled care out-of-hours for patients who may be at the end of life, or experiencing an urgent need for healthcare. This may include treating acutely unwell people that would otherwise require hospital admission, and also supports hospital discharges.
The service covers 24 hours a day 365 days a year. This service is providing nursing care within patients' homes and covering West Kent, the ability to drive is essential to this post.
Under the umbrella of Urgent Care is the Home Treatment Service, which provides hospital level care at home. Part of this role would be supporting the HTS team
