Job Description

An exciting and new opportunity has arisen to join the Carlisle Rural Primary Care Network (PCN) team based with Brampton Medical Practice.
This role will work with our practice clinical team and PCN Frailty Nurses to deliver high level services to our patient population of around 16'700 patients.
Main duties of the job
We are able to offer up to 37.5 hours per week but part part hours can also be considered.
Hours of work will be on a rota basis working between 7.45am and 7pm with Saturdays on a team based rota.

We are a semi-rural dispensing and training practice led by 6 dedicated GP Partners, Business Manager and Senior Management Team serving a population of around 16'700 patients
We operate over 4 sites, 2 of which are based in Brampton, 1 at Wetheral and 1 at Corby Hill

Job responsibilities
Supporting the PCN Frailty Nurse team in their duties to deliver housebound and care home services
Supporting the PCN Frailty Nurse team in their duties to deliver care to patients under their patient list within practice clinics
New patient health checks /NHS health checks / Weight Management
ECG recording
Phlebotomy
INRs Warfarin Management
Assist with Learning Disability and Mental Health annual reviews
To give flu vaccines / immunisations within the scope of established protocols
Chaperoning duties
Processing and management of laboratory samples requested by GPs/nurses
Maintenance of surgical equipment
Cleaning of clinical areas within remit of role
Vaccine/cold chain storage, monitoring and recording
Surgical equipment and vaccine re-stocking and stock rotation
Clearing and re-stocking consulting rooms
Preparing and maintaining environments and equipment before, during and after patient care interventions, including assisting GPs during the performance of minor operations
Assisting in the assessment and surveillance of patients health and well-being
Undertaking specific clinical activities for named patients that have been delegated and taught specifically in relation to that individual
Helping to raise awareness of health and well-being and how it can be promoted
Assisting with the collection and collation of data on needs related to health and well-being
Follows aseptic technique in removal of sutures, clips, dressings as per nursing care plan.
At all times, observes infection control procedures and protocols to protect patients, colleagues and themselves.
Please Note: The successful applicant will be subject to an enhanced DBS Check, the cost of which will be paid by the post holder either on appointment or as part of the first 3 months salary deductions. Current cost is 49.50
