Village Surgery is seeking a Healthcare Support Worker to work across our Primary Care Network.
North Bournemouth Primary Care Network consists of 4 GP Practices, Banks and Bearwood Medical Practice, Kinson Road Medical Centre, Talbot Medical Centre and Village Surgery. It cares for approx. 58k patients across the area.
The Healthcare support worker will provide support with health promotion and prevention activities across the PCN, which include phlebotomy, long term health condition monitoring and reviews, health checks, some injections and vaccinations and offering patients lifestyle advice.
The post holder is required to have experience in health promotion, prevention, and supporting long-term condition management and have completed the Care Certificate or equivalent qualification.
Duties
Phlebotomy
Long Term Condition reviews
Vaccinations and Injections under Patient Specific Direction
Weight monitoring
Blood Pressure monitoring and ambulatory monitoring
Health checks
Urinalysis
Provide lifestyle advice to patients making any necessary referrals within the Practice
Health checks
Ensure specimens are labelled and bagged ready for collection with the necessary paperwork completed
Restocking of clinical areas and consulting rooms where necessary
Participate in the administrative and professional responsibilities of the practice team
Ensure accurate and legible notes of all consultations and treatments are recorded in the patients notes
Ensure the clinical computer system is kept up to date with accurate details recorded
Ensure the clinical equipment is maintained with accurate product and calibration details recorded
Ensure accurate completion of all necessary documentation associated with patient health care and registration with the practice
Administrative work relating to the ordering of stock, room stocking, equipment checks, read coding and summarising where directed
Ensure collection and maintenance of statistical information required for regular and ad hoc reports
Attend and participate in practice meetings as required
Assist in formulation of practice philosophy, strategy and policy
To assist in seasonal and special projects as requested e.g. flu campaign
To attend Practice meetings as requested.

Health & Safety:
The post-holder will assist in promoting and maintaining their own and others' health, safety and security as defined in the practice Health & Safety Policy, the practice Health & Safety Manual, and the practice Infection Control policy and published procedures. This will include:

Qualification and experience requirements
The post-holder is required to have:
Completed the Care Certificate or equivalent qualification.
Experience in health promotion and prevention
Experience in supporting long-term condition management
