Your role will include facilitating safe hospital discharge, preventing readmissions, running cardio-pulmonary rehabilitation programmes, optimising medication, and supporting advanced care planning. Taking a holistic approach, you will address physical, psychological, and social needs, helping patients and their families live as well as possible.
You will also share your expertise through staff education and be supported to complete non-medical prescribing training, with progression to Band 7 on successful completion.

Main Responsibilities To provide appropriate treatments aimed at providing symptomatic relief and improving quality of life, including medication and oxygen therapy monitoring, and where possible, reduce disease progression.
To support the delivery of Community Cardio-Pulmonary Rehabilitation (PR) throughout Wiltshire.
Working as an autonomous practitioner in partnership with other care professionals and using advanced skills and expert knowledge to assess, monitor and review the complex physical, social and psychological needs of the patients with respiratory problems. Develop care plans and instigate therapeutic treatments based on comprehensive patient focused assessments and best available evidence in order to improve health outcomes.
To support community integrated teams to manage a patient's on-going care in accordance with the agreed care plan. This will include but not exclusively; case review, 1:1 supervision and joint visits.
Ensure a safe environment for the treatment of clients and protection of staff through risk assessment and by ensuring the effective use of equipment and materials in compliance with health and safety and other relevant policies
To work as a non-medical prescriber (where competent and qualified) within the locally developed NMP Prescribing Guidelines and NPC national single competency framework. Linking where appropriate to other professionals involved in medicines management such as GP's and Pharmacists.
Promote patients to self-manage their condition and encourage behavioural change strategies by identifying where non-pharmacological interventions are appropriate, e.g. smoking cessation, Cardio-pulmonary rehabilitation, home exercise regimes and improved concordance with medication.

The Ideal Candidate
Essential
RGN or equivalent AHPclinical qualification
Degree or masters or willingness to work towards
Evidence of relevant and on-going professional development
Heart failure diploma or equivalent M level module
Experience of long term condition case management
Experience of working in collaboration with other professionals across organisational boundaries.
Leadership experience
Basic IT skills; experience of electronic patient records
Ability to demonstrate effective verbal and written communication skills.
Ability to co-ordinate and direct programmes of care working collaboratively and autonomously.
Mentorship skills

Extensive experience of working with older people or those with long-term conditions, Eg Respiratory Diabetes .

Recent experience of working in the primary care setting
