Job Description

The Hospital at Home service is an innovative and pioneering service based at the Abingdon Community Hospital in Oxfordshire, covering a large catchment area across Central, West and South Oxfordshire.



The service works collaboratively with Services from the OUH, to provide a holistic approach to assessment, and provide hospital level treatment for patients in their own homes, thus avoiding admission or expediating discharge from secondary care providers.



We are seeking autonomous enthusiastic, and patient focused Practitioners to join our multi disciplinary team, as Non Medical Prescribers, to further develop and allow the H@H Team to offer hospital level care within patients own homes.



The service runs 08.00 - 22.00 7 days a week, 52 weeks of the year. Flexibility within these hours will be required.



Working with a high degree of autonomy, the post holder will:



Manage undifferentiated and undiagnosed conditions by using, developing and consolidating advanced clinical skills within their own sphere of competence.



Support remote management of patients conditions, who otherwise would need to be transferred to an acute care setting to receive assessment and treatment.



Provide holistic patient care, in the patient's residence, from assessment of presenting condition through to diagnosis, hospital level treatment and then discharge, including the prescribing and administration of medication, thus providing a alternative to admission into acute care services.



Work collaboratively within the wider multidisciplinary team and with external providers, to ensure the right care is provided, by the right person, at the right time and in the right place.
