You will be responsible for enrolling new service users, establishing relationships, determining needs, writing detailed care plans, liaising with other healthcare professionals, evaluating progress, handling complaints efficiently, following CQC guidance, supporting care workers and ensuring all care calls are covered, achieving realistic targets.
**Health and Social Care Qualification
**Online DBS (Desirable)

This role is part-time with an hourly rate starting from £10.00ph. Your office is situated at Litchurch Plaza on Litchurch Lane in Derby.
