1.0 WTE - 37.5hours per week



Community Neurorehabilitation



We have an exciting opportunity for a committed, highly motivated, enthusiastic and experienced Occupational Therapist, to join our growing multidisciplinary specialist Community Neurorehabilitation Service. This post offers the opportunity to develop your clinical and leadership skills, as you will be a deputy to the part-time clinical specialist. You will be responsible for supervision of the team as well as having a key role in promoting evidence based practice and service development. We are an innovative team who are a passionate about the specialist service we provide offering skilled development in both stroke and long terms conditions.



All applicants must have experience of working with an adult client group in neurological rehabilitation at a senior level. You need to be a confident autonomous practitioner with high level leadership and communication skills, a flexible problem solving approach, good clinical reasoning skills, and be efficient in your time and people management. The team provides a service in clients' own homes, care homes and the local community and provides a great opportunity for joint working, joint goal setting and regular in-service training.



To provide advanced specialist Occupational Therapy input to clients as part of the specialist Community Neurorehabilitation service.



To provide clinical leadership in partnership with the Principal OT into the day to day delivery of Occupational Therapy in the specialist Community Neurorehabilitation service, ensuring the highest possible standards of practice. This includes supervision and support of junior staff and students.



Support the Team Manager - Specialist Community Neurorehabilitation, in establishing and monitoring a robust clinical governance framework for the specialist Community Neurorehabilitation service.



We provide our service to people in the place most suited to their current need, (their home, day centre, place of work etc) as well as in our own small outpatient gym.



It is also essential that you hold a full UK driving license and preferably have access to a suitable vehicle for business purposes, although Oxleas offers a Lease Car Scheme to all substantive employees.

To be able to work alone in the community in a variety of settings, independent of multi-disciplinary support whilst in direct contact with clients and their carers



To be responsible and accountable for occupational therapy assessment, diagnosis and treatment of highly complex neurological conditions including stroke, as an autonomous practitioner and in partnership with the client / carer



Utilise effective clinical reasoning skills and evidence based practice to develop individual specialist rehabilitation management care plans which address the client's goals and clinical need.



Use a range of highly specialist clinical assessments and skills to ensure effective treatment



To monitor, review and continually re-assess clients with complex care plans and objectives, and facilitate modification of packages of care/targets as appropriate to enable achievement of their goals



To carry out clinical risk assessment and complete risk management plans relevant to the clinical group



To engage in joint patient assessment/ treatment with more junior staff as need dictates



To work in a co-ordinated manner, communicating effectively with other team members and the wider clinical network involved in client care



To provide advice and education to client, carers and relatives based on clinical evidence and identified need



To provide highly specialist advice & second opinions to more junior colleagues/others regarding the management and care of patients/clients with physical difficulties



To maintain up-to-date clinical written records and electronic database in accordance with local and professional standards



To write comprehensive discharge and progress reports to medical referrers.



Triage referrals into the service to ensure the waiting time for assessment is met, and allocation to appropriate skilled staff.



To assess for and arrange provision of rehabilitative equipment as required by client.



To identify and manage where appropriate the mental / emotion health of the client



To ensure the vocational / occupational needs of the client are effectively addressed in treatment/rehabilitation as appropriate
