We are seeking compassionate and dedicated Home Care Assistants to provide exceptional in-home care services to clients across various needs. Responsibilities
Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
Provide medication reminders and assist with medication administration following prescribed care plans
Monitor and record vital signs such as blood pressure, pulse, and temperature
Support clients with meal preparation, feeding, and nutritional needs
Perform light housekeeping tasks such as cleaning and laundry to maintain a safe and comfortable environment
Assist with transfers using equipment like Hoyer lifts or other mobility aids
Observe and report any changes in client health status or behavior to supervising healthcare professionals
Support clients with cognitive impairments including dementia or Alzheimer's care, ensuring safety and engagement
Follow HIPAA guidelines to protect client confidentiality at all times
Implement individualized care plans tailored to each client's specific needs
Skills
Prior experience in home & community care, assisted living, nursing home, or long-term care preferred
Knowledge of patient observation, patient monitoring, and vital signs measurement
Certifications such as First Aid and CPR are highly desirable
Experience working with people with developmental disabilities, autism, or dementia enhances effectiveness in this role
Ability to perform heavy lifting safely when necessary for transfers or mobility assistance
Strong communication skills for effective interaction with clients and healthcare teams
Compassionate attitude with patience and empathy for diverse client needs
Familiarity with HIPAA regulations to ensure privacy and confidentiality in all interactions
Ability to work independently while following established care plans and protocols
