Responsibilities
Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
Monitor patients' health by observing vital signs and reporting any changes to healthcare professionals
Administer medication following prescribed care plans while maintaining compliance with HIPAA regulations for confidentiality
Prepare nutritious meals tailored to dietary needs and assist with meal planning and cooking
Support patients with mobility using equipment such as Hoyer lifts or resident's personal assistive devices
Conduct patient observation to ensure safety and comfort, noting any concerns or unusual behaviors
Perform light cleaning tasks to maintain a safe and hygienic environment in clients' homes or group settings
Assist with transportation to appointments or community activities, promoting social engagement and independence
Follow established care plans meticulously, documenting care provided and patient responses accurately
Support clients with specialized needs such as dementia, Alzheimer's, autism, or developmental disabilities through tailored approaches
Engage in ongoing training on CPR, first aid, hospice care, and other relevant certifications to ensure high-quality support
Collaborate effectively with healthcare teams, social workers, family members, and other caregivers to coordinate comprehensive care
Experience
Prior experience in home health care or assisted living environments is highly preferred
Demonstrated knowledge of patient monitoring techniques and vital signs assessment
Experience working with individuals with developmental disabilities, dementia, Alzheimer's disease or memory care needs is advantageous
Familiarity with nursing home procedures, long-term care practices, and hospice or palliative care settings is beneficial
Proven ability to perform heavy lifting safely using equipment like Hoyer lifts when necessary
Strong understanding of HIPAA regulations to ensure patient confidentiality at all times
Experience in meal preparation and medication administration within a caregiving context is desirable
Ability to work effectively in group home settings or community-based environments with diverse populations
