We are recruiting enthusiastic, caring, strength based; person centred Home First/Reablement/Crisis Response Support Workers.
We work collaboratively, 24/7. It is crucial the team has the right people, with the right skills, behaviours, attributes and core values to support adults and their carers to achieve independence as far as possible.
Previous experience is desirable however not essential, as full training will be provided, including Q.C.Fs in Health and Social Care and a range of other accredited courses to further your career opportunities.
We know that a Home First/Reablement approach discharging people from hospital in a timely manner, back to their own home encourages a quicker recovery and the person's wellbeing and long-term needs are much more positive. By establishing daily living skills (bathing, dressing, medication administration, continence care, and nutrition) and promoting community connections through a time limited, goal orientated programme, people remain in their own for longer and prevent hospital/residential care admission.
Additionally, you will have the opportunity to be involved in supporting the Borough's domiciliary care services and Nursing/residential homes in times of need.
You will be part of a multi-disciplinary team including Social Care Professionals, Therapists, Voluntary and Community Services to provide short-term prevention and rapid response intervention to enable the adults we support to live to their maximum level of independence and achieve their goals.

Support Workers work on a rota basis and are required to be flexible in their working hours subject to the needs of the service, which includes weekends, evenings, nights and Bank Holidays.
It is a requirement that you have a car available for work and are willing to work anywhere in the borough subject to the needs of the service. For using your car in this role you will be paid for your mileage.
