About this exciting new Role



Access Your Care is seeking a dedicated and proactive Homecare Coordinator to join our passionate team supporting individuals across North Somerset. This is a vital role at the heart of our operations, ensuring high-quality administration around care delivery and seamless coordination of services.

You'll be the first point of contact for incoming calls, triaging needs, supporting care staff, and auditing service delivery to maintain our exceptional standards. If you thrive in a fast-paced environment, have excellent administration skills and have a genuine passion for health and social care, we'd love to hear from you.

Key Responsibilities



Monitor daily service delivery Champion excellence in administrative processes by ensuring accurate, high-quality records and documentation are consistently maintained Triage incoming phone calls with professionalism and empathy Audit care services to ensure compliance and quality control Communicate effectively with clients, families, care staff and external professionals Support the team during the 8-week manager-on-call rotation Support the coordination team with assessments and reviews in the community, when required Delivering hands on care (including personal care) when the need arises in an emergency situation
