Job overview




Internal CFT - open to existing B6 staff within the department





The HomeFirst team comprises registered and non-registered nursing, therapy and administrative staff, working collaboratively with partner organisations to support the intermediate care needs of our local population.



This position is open to nurses, therapists, or paramedics with experience of working in the community and experience of undertaking assessment, care planning and reablement as part of a dynamic, integrated, multiagency, multidisciplinary team.



You will be responsible for assessment, planning and implementation of care, and assist in the interpretation of information relevant to the patient's condition/progress, identify problems and needs and set realistic goals. You will need excellent organisational skills along with a wide knowledge base and experience of working in older person's services.



We will offer a working environment in which to develop your clinical and interpersonal skills, a structured in-service training programme; appraisal and supervision system.



We are based in Falmouth and cover the Carrick area including Falmouth, Penryn, Truro, Perranporth, and surrounding areas.


This is a 22.5 hour post and days are planned over a 7 day rota which involves regular weekend working and some bank holidays: the successful candidate must be flexible with their working days. We cover 8am-6pm.

Main duties of the job




You will need to be able to a triage, assess and provide evidence based treatment plans for adults within the catchment area, either in their own homes or nursing and care homes.



To work alongside the wider multidisciplinary teams within the community, liaising with the community and acute hospitals, Adult Social Care and the Voluntary Services.

Degree or diploma in Occupational Therapy or Physiotherapy, Nursing HCPC or NMC registered Substantial post registration clinical experience in a similar role Extensive experience of working within a multidisciplinary team Experience of managing the risks associated with working in a lone working environment Evidence of appropriate postgraduate courses and CPD to support practice, to degree equivalent level A broad range of clinical experience in a variety of community and inpatient settings ####
