What are we looking for?
We're looking for a highly motivated, passionate Registered Nurse to join our expanding Hospice at Home (H@H) Team at St. Michael's Hospice. This is a specialist nursing role, providing compassionate, expert care to patients in the last weeks and days of their lives.
This is an ideal opportunity for a nurse who wishes to specialise in palliative and end of life care, or who already has experience in this area. Passion for making a difference to our patients and their families alongside a love of the nursing role are key aspirations we are looking for in the successful candidate. Experience of working within the community setting would be an advantage.
The successful candidate will be an NMC-registered nurse. You will have excellent communication skills and empathy. You'll work both independently and as part of a supportive multi-disciplinary team, proactively assessing patients' needs, planning and delivering high-quality care, and advocating for the best possible outcomes.
You will have the confidence and knowledge to make clinical judgements, manage your workload effectively, and deliver care that aligns with St. Michael's Hospice's values and commitment to excellence.
This role involves working shifts across long days, including both day and night shifts. A full UK driving license and access to a vehicle are essential, as you will be travelling across the community within the hospice's catchment area.
The role
Working collaboratively with the wider multi-disciplinary team, you'll assess, plan, and provide holistic care that meets the physical, emotional, and spiritual needs of patients. You'll also play a key role in contributing to service improvement and promoting best practice in end-of-life care.
Key responsibilities:
Deliver personalised palliative care and symptom management at home.
Support the SMH Living Well Service, including MND and long-term neurological conditions.
Maintain high standards of care, communication, and record-keeping.
Collaborate with healthcare professionals, patients, families, and carers to ensure a seamless, compassionate service.
