About the Role
This is a rewarding role supporting individuals as they return home from hospital, helping them settle back in safely and avoid unnecessary readmissions.
You'll provide short-term, practical support such as help with shopping, light household tasks, and ensuring people feel comfortable and confident at home. At times, you may also offer gentle, hands-on assistance with day-to-day routines to make sure people are safe, well, and able to regain their independence.
Every person we support is unique, so no two days will be the same. The role involves a mix of planned support and responding to new referrals promptly, making it ideal for someone who enjoys flexibility and variety.
Key Responsibilities
Support safe and timely discharges home from hospital.
Provide short-term practical assistance (shopping, light cleaning, essential household tasks).
Offer reassurance and hands-on support to help people with everyday routines and confidence at home.
Ensure people have the basics they need at home (food, heating, lighting, hydration).
Work alongside healthcare teams to coordinate smooth transitions.
Connect people with longer-term services for ongoing support.
Maintain accurate records in line with policy and quality standards.
Travel between homes to deliver support.
Promote Age UK Gateshead's services within the community.
What We're Looking For
We're seeking someone who is:
Compassionate, reliable, and passionate about supporting older people.
Flexible and able to adapt to changing needs and priorities.
Comfortable providing practical and, when needed, gentle hands-on help with day-to-day routines.
A good communicator, confident working with people facing multiple health challenges.
Organised, with the ability to manage a busy schedule.
Respectful of confidentiality and professional boundaries.
Able to work both independently and as part of a team.
Desirable but not essential:
Experience in care, support work, or health & social care.
A relevant qualification in Health & Social Care.
Access to your own vehicle.
