Our Hospital to Home service helps patients who are well enough to leave hospital and continue their recovery at home with appropriate care.

Key Responsibilities:
Assist individuals with medication management and maintaining accurate records
Assist with personal care tasks, including bathing, toileting, and dressing
Prepare and serve meals according to dietary requirements and assist with feeding when necessary
Offer companionship and emotional support, engaging with the people we support in meaningful activities and conversations
Maintain accurate and detailed records of care provided, daily observations, and any changes in individuals conditions
Ensuring all interactions with individuals are conducted with the utmost respect, preserving their dignity, privacy, and choice
Promote and support individuals in maintaining their independence and self-esteem
Work in multidisciplinary teams to ensure the holistic well-being of the individuals we support
Essential Requirements:
Previous experience in domiciliary care or a related field
Strong communication and interpersonal skills
Valid driving licence and own car
Good fluency in English
Proactive and problem-solving approach
Empathy, patience, and a genuine desire to improve the lives of others
The ability to work independently and as part of a team
Reliable and flexible availability to work evenings, weekends, and bank holidays.
Benefits:
Pay starting at 12.38 per hour after probation
Enhanced pay on evenings, weekends and public holidays

Desirable Requirements:
NVQ/QCF Level 2 or 3 in Health and Social Care (or working towards).
