We're looking for passionate and dedicated people with a talent to work with vulnerable adults with a wide range of issues.


Role Overview





This role supports the resettlement of individuals with a rough sleeping history who are hardest to reach by facilitating access to accommodation. The position involves close collaboration with lead workers, housing providers, and statutory/non-statutory agencies to ensure a smooth transition into stable housing.


Key Duties & Responsibilities




Build and maintain strong relationships with accommodation providers. Monitor bed space availability and match clients appropriately. Ensure timely and suitable offers of accommodation are made. Collaborate with lead workers to ensure all necessary steps are completed before move-in: Confirm or initiate homeless applications. Coordinate with housing options teams and ensure up-to-date information is shared. Assess the need for social care involvement. Ensure support and risk assessments are current to avoid duplication. Match clients to accommodation based on individual needs and preferences. Ensure a personalized approach to maximize the chance of successful resettlement. Continue engagement post-move-in to monitor progress and identify additional support needs. Case Management (When No Other Services Are Involved) Take on the role of case holder. Refer clients to appropriate services to build a support network. Conduct needs and risk assessments. Support clients in developing skills and confidence to live independently. Promote safety, security, and well-being throughout the resettlement process. Participate in meetings and contribute to data analysis. Work collaboratively with other agencies to enhance service delivery.
