We are seeking dedicated and compassionate Hub Facilitators to join our team in the North Day Opportunities at Newark and Bassetlaw. The successful candidate will be responsible for ensuring the provision of day opportunities for people and meeting their care and support needs within our practice framework. You will work within a geographical area and be prepared to work at different buildings across the district.
You must provide high-quality physical and emotional support and personal care to individuals, respecting their privacy, dignity, and specific needs. You must support individuals within a risk management framework, ensuring their personal welfare and ensuring escalation of any risks or unresolved issues. The Hub Facilitator will develop care plans based on effective consultation with individuals, carers, specialist workers, and other relevant agencies and meet individuals' care and support needs appropriately and respectfully. You will work on a 1:1 basis or with groups of individuals at a base or in the community.
The Hub Facilitator will provide cover and assistance in respect of staff absence and other requirements as appropriate within a geographical area. Also use any equipment as directed by the care plan, following appropriate training. They must report any health and safety issues relating to premises, vehicles, equipment, and other facilities used by day opportunities.
The Hub Facilitator will ensure effective delivery of day opportunities activities within departmental policies, legislation, and practice standards and Support individuals to stay in control, promoting, developing, and maintaining independence. They will build relationships with individuals, carers, and partners to write, review, and monitor care plans and risk assessments.
They will undertake medication administration and recording procedures in line with the medication policy. The hub facilitator will respond appropriately to crisis and emergency situations and report and escalate incidents. They must ensure that they collaborate with stakeholders and contribute to reviews, meetings, and other forums as required. Effective supervision of staff.
The successful candidate must have:
A Social Care qualification or equivalent, with a care qualification or working towards Level 3 Diploma in Adult Care (with a supervisory role).
Knowledge of relevant legislation.
Evidence of continuous mandatory training and development.
Knowledge of main ICT systems in the service area.
Excellent professionalism, efficiency, and effectiveness in service delivery.
We have full time and part time vacancies, Bassetlaw - 2 x 37 hours, 1 x 32 hours, Newark - 1 x 37 hours.
