We are currently recruiting for City Health Care Partnership CIC who are looking for Band 6 Community Physiotherapist to join the team.

Role Overview:
As a Band 6 Community Physiotherapist, you will play a crucial role in delivering patient-centered care within the community setting. You will assess, plan, and implement effective physiotherapy interventions, working collaboratively with multidisciplinary teams to achieve the best outcomes for our service users. This role offers an exciting opportunity to contribute to the improvement of peoples lives and well-being in a supportive and progressive environment.

Pay Rate: Dependent on experience

Key Responsibilities:
Conduct comprehensive assessments and develop tailored treatment plans for individuals in the community.
Provide evidence-based physiotherapy interventions and education to promote self-management and independence.
Work closely with other healthcare professionals to ensure a holistic approach to patient care.
Maintain accurate and timely records, ensuring compliance with professional standards and organizational policies.
Contribute to service development and quality improvement initiatives.

Qualifications and Experience:
HCPC registration as a Physiotherapist.
Minimum of 2 years post-qualification experience.
Experience in community physiotherapy is highly desirable.
Strong communication and interpersonal skills.
Ability to work independently and as part of a multidisciplinary team.
