Job Summary:
We are seeking a dedicated Case Worker to join our team in providing support and assistance to individuals in need. The successful candidate will play a crucial role in advocating for clients and ensuring their well-being.

Responsibilities:
- Conduct assessments to determine clients' needs and develop individualised care plans
- Provide guidance and support to clients in accessing relevant services and resources
- Monitor and evaluate client progress towards set goals
- Maintain accurate and up-to-date case notes and records
- Liaise with external agencies, such as social services and healthcare providers, on behalf of clients
- Advocate for clients' rights and negotiate on their behalf when required

Requirements:
- Proven experience working in a similar role within the social services sector
- Strong communication and interpersonal skills
- Ability to empathise with clients from diverse backgrounds
- Excellent organisational skills with the ability to manage multiple cases simultaneously
- Knowledge of relevant legislation and policies governing social work practice
- A compassionate and patient approach to supporting vulnerable individuals
- A recognised qualification in social work or a related field
