RAD has a fantastic opportunity for an
Independent Living Coordinator
to join its Independent Living Service.
The Independent Living Coordinator is responsible for supporting RAD's Independent Living Service, with a primary focus on administrative support. This role involves reviewing client files, support plans, risk assessments, coordination of assessments and meetings whilst also bridging the gap between client service packages, service delivery teams and the case management system.
About the role:
This can be either:
A hybrid role at 23,809.50 full time equivalent (with some travel to London Hub expected)
Or
Based at our London Hub base at 25,642.50 full time equivalent
This is a permanent role, working part time 22.5 hours per week and we would be open to flexi working within this role.
Responsibilities as our Independent Living Coordination will include:
Supporting RAD's overall Independent Living Service:
Client Case Files:
Review all client files, support plans, and risk assessments to ensure accuracy and completeness.
Administrative Service Coordination:
Bridge the gap between client service packages, managers, service delivery teams, and the case management systems to ensure seamless service delivery. Ensure appropriate meetings are scheduled such as review meetings. Oversee and management of email accounts and first port of contact email inboxes.
Case File Implementation:
Implement and monitor associated case files, ensuring they are kept up to date in line with service and client care package reviews, and that observations, feedback and case studies are carried out.
Data and rostering:
Support with rota planning, data inputting and extraction. Supporting data and narrative reports.
Signposting Services:
Make recommendations to signpost clients to other RAD services, such as Advocacy or gateway services, where appropriate.
Service standards:
Coordinate activities associated with this role in line with quality standards, contractual requirements, and strategic ambitions, continually seeking to improve services and performance.
Essential requirements we're looking for in our Independent Living Coordinator:
Willing to attend any training as appropriate to the post
Administrative qualifications
Safeguarding qualifications
Experience of working with deaf people or within the deaf community and an understanding of deaf awareness and deaf culture
An understanding of the barriers faced by deaf people
Experience of working collaboratively as part of a team - bringing people together to achieve positive outcomes
Experience of care service administration
Experience of case management, administration, rostering and care service provisions
Diplomacy and sensitivity in handling difficult situations and conflict
Knowledge of databases/recording systems and a competent user of IT systems
Ability to multi-task
Caring and compassionate
Proactive
Knowledge of safeguarding
Confidently able to advocate the needs and wishes of the people who we support
Proactive and creative approach to the development and delivery of person-centred support
Flexible in approach to working including evenings and weekends or bank holidays if required
Liaison and networking
Must undergo an enhanced DBS check
