We have an exciting opportunity for a Integrated Care Nurse to join our Planned Care team at HMP Pentonville, a

category B

Prison located in

London.





HMP Pentonville is a male prison that has a capacity for 1200 inmates, it is a remand prison for inmates awaiting trial. Well situated in London and is 10 minutes from Kings Cross station.



They have an on-site staff canteen and gym, the training and development opportunities are fantastic and this site is well invested into their staff in this area.


Hours





37.5 hours a week working Monday to Friday, 7.30am-4pm. Some flexibility required.



Salary



This position is full time, you'll receive an annual salary up to 52,000per annum FTE - depending on experience.

What you'll be doing





As a Integrated Care Nurse your responsibilities will vary. You will


A Physical Health Nurse working in mental health settings plays a vital role in bridging the gap between physical and mental healthcare. You will assess and manage the physical health needs of individuals with serious mental illness, often collaborating with multidisciplinary teams to deliver holistic, evidence-based care. Monitoring vital signs, coordinating clinics, supporting staff training, and promoting health awareness to reduce health inequalities and prevent diagnostic overshadowing. Undertake medication rounds, dispense controlled medication, attend emergency response situations, assess patients, and undertake reception screening Use your skills and knowledge to deliver nurse led clinics to your patient group whilst being supported by a multi-disciplinary team. Provide outstanding evidence based care to our patients, seeking out, listening to and acting on their feedback, so that care is personalised and informed by what matters to them.

Qualifications required:




Registered nurse - Adult Primary Care experience, Practice Nurse or Mental Health experience - desirable Current professional qualification (NMC) Evidence of Continued Professional Development Understanding of the complexities of multi-disciplinary and multi-agency working
A non-judgmental and compassionate approach

Although not essential ideal applicants would have experience in a hospital, community or prison environment.
