Integrated Care Pharmacist, Community Independence Service (37.5 hours); Band 8a; 9 Months fixed term contract (Maternity leave cover)



We have an exciting opportunity for a highly motivated, enthusiastic pharmacist with excellent clinical skills to join our Community Independence Service (CIS).



The post-holder will provide a clinical pharmacy service to CIS as well as lead on medicines optimisation (MO).



The CIS service aims to offer safe care at home which enables people to avoid unplanned hospital admissions. The pharmacy role is pivotal in this to reduce the risk of avoidable admissions due to medication.



Central to your role will be carrying out individual assessments in patients with complex medicines needs or with an identified medication issue. This will require travelling to the patients' home to carry out the assessment. You will work in close partnership with other members of the CIS team, in order to provide high quality patient care.

Lead on the delivery of a clinical pharmacy service across Community Independence Service (tri-borough).

Be the source of expert clinical advice for medicines information related queries.

Carry out domiciliary assessments and medication reviews in identified high risk patients with complex medicines needs.

Formulate medicines care plans jointly with the patient and/or carer.

Communicate interventions with the patient/carer, their GP and other key health professionals involved in their care.

Provide professional advice and support on the safe prescribing, handling and administration of medicines.

Collaborate with internal and external stakeholders to improve and develop robust medicines optimisation pathways.

To manage and provide clinical supervision to Pharmacy Staff within designated area.

Work with GPs and participate in MDTs

Lead on service evaluation and monitoring



Frequent continuing education events open to all pharmacy staff across CNWL will allow you to network across the Trust.

The post holder will be responsible for providing, leading and developing a medicines optimisation (MO) service to CIS within Kensington & Chelsea and Westminster boroughs. They may also provide to support and MO leadership to Hammersmith & Fulham CIS team when needed.



The overarching aim is to support patients recently discharged from hospital with their medicines in order to improve safety, adherence to treatment, reduce waste and to support patients in the community to prevent avoidable readmissions due to medicines and improve MO related patient outcomes. They will also lead on developing systems to facilitate the efficient, safe and cost -effective use of medicines, working across the interfaces to improve MO.
