We hold NHS contracts supporting primary care delivery, driving service improvement, and helping practices work collaboratively with wider health and social care partners. Our services include Integrated Care Teams (ICTs), the Community Single Point of Access (CSPA), and a growing portfolio of innovative programmes.
Job Summary
The Administrator will play a vital role in supporting the delivery of care across our model - a wide-ranging portfolio of community-based services that includes triage, multidisciplinary team coordination, proactive frailty pathways, hospital discharge, and holistic patient support.
You will provide high-quality administrative support across both the Integrated Care Team, the Paramedic Home Visiting Team, and the Community Single Point of Access (CSPA), working as part of a collaborative 5-person administration team.
You will support both locality-based delivery and cross-cutting service functions with a defined portfolio of work tasks. You will be involved in frontline communication with clinicians, partners, and occasionally patients and carers.
Objectives
To work flexibly across ICT, Paramedics, and CSPA, contributing to efficient service delivery
To support coordination and communication with internal teams and external partners
To provide cover and capacity across the admin team during periods of leave or increased workload
To contribute to service development projects and quality improvement initiatives
Key Responsibilities
CSPA (Community Single Point of Access)
Process incoming referrals and document outbound referrals
Liaise with triaging clinicians to ensure all actions are accurately recorded and followed up
Manage and oversee the single need caseload for the referrals hub, ensuring referrals have reached their destinations in a timely manner, reporting any missed referrals as significant events
Act as a point of contact for operational CSPA queries
Manage upkeep of the CSPA office, including ordering supplies and maintaining shared resources
Maintain contact directories for key partners and services
Participate in quality checks to validate referral activity
ICT (Integrated Care Team)
Support maintenance of the ICT caseload including records entry and partner communications
Provide support to ICT clinical staff in managing daily operational issues
Administer shared calendars, meeting bookings, and diary management for colleagues, as required
Promotion and monitoring of the ICT patient feedback
Auditing support, as required
Paramedic Administration
Support with student placement scheduling
Support with referral form completion
Ad-hoc administrative support, as required
General Administrative Support
Support administrative elements of departmental meetings and briefings
Handle telephone communications on behalf of ICT/Paramedic/CSPA including referrals hub line
Maintain operational supplies and IT resource availability
Support coordination of team rotas in collaboration with the wider admin team
Escalate office, IT, or building issues to the relevant leads
Project and Programme Support
The NHS landscape is continuously evolving, and you will support the delivery of affiliated projects as required.
You may also contribute to the development and implementation of new protocols, and service evaluation activities.
Skills & Experience
Prior administrative experience, preferably in a healthcare, public sector, or multi-agency setting
Excellent communication and interpersonal skills
Proficient in Microsoft 365 (Teams, Outlook, Excel, Word)
Able to prioritise and manage competing deadlines
Flexible and proactive, able to work independently and as part of a team
Confidence in working with confidential data and clinical systems (full training provided)
Driving licence and access to a car required for occasional travel
This post requires that business use is added to your vehicle insurance, as travel between base locations is a core part of the role
