Job overview

We are seeking an experienced and motivated

Integrated Community Manager

with an Allied Health Professions background to lead our Community Health Teams and drive the development of Integrated Neighbourhood Teams across North Cornwall.



This pivotal role focuses on delivering high-quality, person-centred care by fostering collaboration between health, social care, and voluntary sectors. You will provide operational leadership, ensure service efficiency, and champion innovation to meet the needs of our local population.



As part of our commitment to integrated care, you will play a key role in shaping seamless working at "place," supporting teams to deliver joined-up services that improve outcomes and reduce health inequalities.

Main duties of the job

The post holder will manage and be responsible for the delivery of a portfolio of services, line managing staff in the integrated care area and matrix management of staff in neighbouring district. The portfolio will include a range of community services. The Integrated Community Managers with jointly have professional responsibility and accountability for provision in the community across the area with the East Integrated Community Team Manager.

We're an NHS community and mental health provider Trust based in Cornwall and the Isles of Scilly. We deliver community and hospital-based care to improve people's physical and mental health. We also provide specialist support to people with dementia or a learning disability.

Approximately 568,000 people live here. A third of people who live in Cornwall are supported by acute hospital services in Devon. As a result, we also work closely with our partners in Devon. In the summer, and during other holidays lots of people choose to visit the area. This increase the numbers of people who use out services.
