Job description
Part Time / 30 and 24 Hours Per week / Permanent

Why not join LCC as part of our Integrated Crisis Response Service (ICRS) which is rated "Outstanding" for its responsiveness by the Care Quality Commission and recognised as being at the heart of the health and social care system. We're currently recruiting for ICRS Assistants who can help build on the success of the service and also help pilot our new End-of-Life Care pathway across the County.

The 24-7 ICRS Service, provides critical social care support within 2 hours for people experiencing a social care crisis within their own home by working alongside various health professionals, who, without ICRS's support would be admitted to hospital or need an emergency placement. People remain on the ICRS pathway for up to 72 hours after which their needs are assessed for any ongoing support. Key responsibilities include being the first responder to the crisis, undertaking a dynamic risk assessment and engaging, encouraging and motivating the person in stabilising the crisis through a personalised, supportive and caring approach.

As an ICRS Assistant you'll join a team of 30+, which includes front line staff, officers, co-ordinators and seniors.

The positions we have available are part time 30 and 24 hours per week. Those not working nights will work a mixture of morning, afternoon and evening shifts between the hours of 7am to 10pm across a rota covering 7 days a week, this includes weekends, bank holidays and other public holidays. Within both day shift and night shift roles, you'll be based at the Neville Centre on the Leicester General Hospital site.

This role is ineligible for sponsorship. You must have the right to work in the UK prior to applying.

What you'll be doing

As an ICRS Assistant your most integral tasks include supporting people that have been referred into the service across the city within 2 hours and undertaking a dynamic risk assessment to ensure a safe working environment for yourself and the person facing the crisis. In addition to this you'll carry out various health checks where appropriate which includes measuring oxygen levels and carrying out blood pressure and temperature readings. Your responsibilities will include responding to fallers and people on the End-of Life care pathway across the City and the County as part of our integrated offer of support whilst helping contribute towards a personalised support plan for the person when ongoing needs have been identified.

This role also involves identifying any health needs alongside key pieces of equipment and assistive technology as a Trusted Assessor. Excellent communication skills both written and verbal are central to the role to help build relationships both with the people we serve and across our various partnerships within Home First. Providing critical feedback to help determine the next key steps and the amount of time needed in a strengths-based approach is a key part of the role whilst working alongside Home First Services such as Reablement, Nursing, Therapy as part of our integrated and co-ordinated offer of support.

What you'll need

The successful candidates will have experience of working within a health or social care setting which includes the provision of care to adults and older people with complex needs (for example: a physical disability, a learning difficulty or a mental health diagnosis). You'll also understand and have the ability to identify all forms of abuse and report/record this in line with our safeguarding procedures.

You'll be able to communicate effectively to a wide range of people from varied and diverse backgrounds. Using your excellent communication skills both verbal and written sufficient you will write progress reports, whilst ensuring accuracy when completing contact sheets and medication records at all times.

You'll need to be responsive and resilient working under pressure in what is challenging and equally rewarding role whilst having the ability to problem solve in often difficult circumstances.

Overall, you'll be keen to learn and ready to take on new skills within a short period of time in order to consistently perform highly alongside the service to offer a service to people that is safe, responsive and caring.
