Wealden Neighbourhood Mental Health Team, Specialist Older Adult services have two Band 4 vacancies for Support, Time and Recovery Workers (STR Worker). The role will be based at Millwood, Uckfield Hospital, Uckfield and work with a range of multi-disciplinary professionals providing assessment, treatment and interventions including the provision of intensive support, and treatment to enable people to receive care and treatment in their home setting avoiding the need for hospital admissions.

Joining the Trust, you will become part of a network of professional nursing and OT staff working with older adults and people with dementia (and their carers) and have the opportunity to network with, and learn from, colleagues in similar roles across the Trust.

Positively promote the independent living of service users within the community.
Ensure service users engage effectively with the agreed CPA Care Plan and access appropriate services.
Be responsible for liaising with and providing information to other agencies involved in the care of the service user.
Participate in reviews and case conferences and attend hospital ward rounds.
Develop a rapport and a therapeutic relationship with service users, within appropriate boundaries.
Provide regular and practical support to service users and their carers (as appropriate) in developing and managing independence and maintaining dignity and self-respect.
Provide support with daily living, including self care and home management e.g. budgeting, cooking, cleaning, shopping etc.
Help service users gain access to resources to include benefits and welfare rights.
Help service users access community activities, e.g. clubs, centres, libraries and to daytime services.
Liaise on behalf of the service user with other agencies e.g. Benefits Agency, Housing Department, gas and electricity services etc.
Provide information on health promotion.
Help to identify early signs of relapse by monitoring the client's progress, level of functioning and mental state and alert the appropriate staff involved in the client's care.
Maintain adequate records as required by existing policy.
