Job Description

We have an exciting opportunity for a Clinical Team Lead- Nurse to join our Intermediate Care Team in the North of Cambridgeshire and Peterborough.



The Intermediate Care Service provides care and therapy in patients' own homes and in private provider care home settings, facilitating discharges from hospitals and admission avoidance using a home first care model.



The post holder will offer leadership and management to ensure the effective delivery of the Intermediate Care Service across the North locality as part of the countywide Intermediate Care Service.



The post holder will work in collaboration with the Clinical Team Lead- Therapist to deliver high quality, patient-centred care within the Intermediate Care Service.



The successful applicant will need to be innovative, decisive and display strong leadership and organisational skills.



The post holder will be responsible for ensuring the smooth running of the team, maintaining quality standards and maximising resource within the team. In addition, playing a key role within the local system to optimise patient flow, working collaboratively with countywide service colleagues and partner organisations.

Providing leadership and management on matters relating to admission avoidance, early supported discharge, transfer of care process.

Work closely with the MDT and coordinators, integrating working practices to streamline processes, share knowledge, and benefit patient experience, outcomes and flow.

Manage systems to ensure the effective allocation and deployment of staff.

Act as a resource for the team and others involved in the patient pathway, providing advice to facilitate timely discharge and flow through the pathway and with the aim of reducing hospital stays, maximising patient independence, enabling patients to receive the right care at the right time in the right place.

Ensure systems and processes are in place to receive and prioritise referrals to the team and delegation to staff

Ensure own leadership style facilitates effective communication, collaboration and motivation of staff and partners to achieve effective transfer of care.

Work collaboratively with the team and professional leads to support professional and practice developments including CPFT Clinical Supervision framework

To ensure the effective daily delivery of the service in the community responding to needs identified in the community or following hospital admission
