The Intensive Life Skills (ILS) service is looking for an experienced practitioner to join a progressive, enthusiastic and forward-thinking service that supports individuals and their families who are experiencing complex emotional needs in Shropshire Telford and Wrekin.
You will need to be registered with your core profession registering body and have an understanding of how multidisciplinary teams function well and be passionate about delivering and developing a quality service to working age adults and their families.
Our ILS service are going through an exciting development phase and this role will be fundamental in the development of and the continued development of the ILS service. Strong leadership skills will be essential within this role for leading on service projects and improvement initiatives.
The post is full-time on a fixed term basis offering excellent opportunities to develop special professional interests; share expertise and work closely with ILS colleagues. Training, good support and partnership work is offered. Close relationships also exist with multi-agency partners and specialist services with this post is crucial in the maintaining of strong and positive relationships with internal and external services.
We are a county-wide based team and our operational working hours are Monday to Friday 9am to 5pm, with some flexibility when required to engage with our service users.
To provide appropriate advice to team staff, multi-disciplinary and Management Groups.
To promote, encourage and develop integration between Community and inpatient services.
In conjunction with the operational lead, monitor and manage the performance of the team and individual team members, to ensure achievements of Trust organisational targets/objectives.
Ensure compliance with the Trust's objectives in relation to the Quality and Safety Agenda achieving a continuous process of monitoring and improving safe effective care delivery
Maintain a strong overview on environmental standards and address any issues relating to safety, dignity, poor quality and poor hygiene and Infection Control.
Promote service audit, evaluation and research.
To lead the development of service standards and policy, including benchmarking, and identifies areas for potential service improvement.
To promote shared decision making with Service User involvement in all aspects of their care
To ensure the availability of clinical supervision and preceptorship
To act as the pathway link, enabling other pathways to engage support for a service user.
To utilise expert clinical knowledge and nursing skills and /or specific to discipline to provide assessment, intervention and clinical guidance as appropriate.
