What are we looking for?

Cleveland Clinic London are looking to recruit a Lead Case Manager to join our Case Management Team, with a competitive salary range on offer.

This is a Monday to Friday days-only role, generally between 0900 \xe2\x80\x93 1700, with flexibility as per the service requirements.

The Lead Case Manager\xe2\x80\x99s role is a key on the delivery of high quality, cost effective and innovative nursing practice consistent with regulatory and professional requirements. Acting as a visible credible role model, you will create a patient-centred environment within the CCL putting the patient first.

The Lead Case Manager will support the Director of Acute and Ambulatory Care in delivering the Cleveland Clinic London (CCL) key performance targets in relation to quality, patient experience and cost-effective service delivery.

What will your duties include?

Will manage the service, leading the Case Management team and actively influencing and developing a culture of leadership, continuously improving patient experience, care and outcomes.

Identifies opportunities to integrate and innovate care delivery whilst being proactive in seeking out different styles and methods of communication to achieve the best outcome.

Participates in research and innovation and the dissemination of best practice guidelines in Case Management.

To evaluate the outcome of patient interventions and treatment in relation to planned discharge dates. Where there is variations, work with the multidisciplinary team to implement agreed actions to achieve improved outcomes.

Contribute towards and promote the coordination of primary care teams, independent health and social care providers and health and social care services in order to enhance the patient\xe2\x80\x99s quality of life at point of discharge from CCL.

What we need from you?

Bachelor\'s Degree in Nursing required.

Master\'s Degree in Nursing or relevant experience required.

Current registration through Nursing & Midwifery Council (First Level) required

Demonstrable broad managerial experience in the field of integrated discharge planning and/or case management.

Ability to coordinate your workload and supervise members of the team, ensuring high quality, safe and effective patient care.
