You will
As the Lead for Community Nursing, you will play a pivotal role in shaping and delivering our community services. Working closely with patients, families, GPs, and wider healthcare professionals, you will coordinate care that respects dignity, autonomy, and individual choice. You will also support and inspire your team to achieve clinical excellence and innovation.
Your leadership will ensure our services are responsive, efficient, and aligned with the needs of our local community
Provide strategic and operational leadership to the Community Nursing Team.
Lead the delivery of high-quality palliative care within patients' homes.
Collaborate with internal and external partners to promote integrated care.
Support the education, development, and wellbeing of community nursing staff.
Monitor service effectiveness and contribute to continuous quality improvement.
About you
We are looking for someone with...
Registered Nurse with current NMC registration and a First Level degree in a related subject.
Significant experience in community nursing and palliative/end-of-life care.
Proven leadership or management experience within a healthcare setting.
Excellent communication, coordination, and team-building skills.
A passion for holistic, patient-centred care in line with hospice values
