Job Description

Nova Healthcare Solutions Ltd is a CQC registered organisation that provides remote support to primary healthcare services through workflow management and medicines optimisation.

The Lead Pharmacist role is dedicated to leading, planning, organising and delivering clinical services within agreed standards of care. This is achieved in a safe and efficient way within defined limits of responsibility and resources.

Duties involve day to day management and clinical supervision of staff, developing and implementing strategies for service growth, streamlining current processes and services, increasing revenue streams, pro-active meetings with prospective clients and maintaining excellent relationships with existing clients.

The role is fully remote working with the occasional requirement to meet in an office environment. Full training is provided.

Clinical practice

Take ownership for the development and delivery of clinical services and remain accountable for staff and personal professional actions guided by agreed local policies, procedures and professional code of practice. Provide and organise clinical services that will include work on premium & premium+ workflow packages in compliance with national and local standards. Provide and organise clinical services that may include: A systematic approach to individual patient care; and Provision of expertise in clinical medications review; and Reconciliation of medicines following hospital discharge; and Reduction of medicine related hospital admissions and readmissions; and Liaison with secondary care colleagues to streamline referral processes; and Quality and safety of prescribing through audit and PDSA cycles; and Drug withdrawals and alters; and Provision of care in line with NICE and other guidelines; and Provision of clinics; and Multidisciplinary reviews; and Review of Pathology results for patients on known medicines; and Provision of expertise and advice on medicines (e.g. doses, side effects, adverse events, possible alternatives etc); and Minor ailments triage; and Risk management and compliance with medicine legislation; and other services yet to be implemented. Provide advice on drug related clinical governance and risk management issues.
Staff support

To work within set parameters to efficiently supervise staff in all aspects of workflow management and clinical services. To proactively create and deliver regular training materials and sessions to staff in line with company policy and protocols. To ensure that staff form a high performing team and meet all set key performance indicators and targets. To spot check an agreed quantity of staff work to ensure that the highest standards are maintained, flagging, retraining and resolving any issues encountered within a timely manner. To ensure that records are up to date and accurate. To ensure staff are compliant with mandatory training. To take a pro-active approach towards recruitment, induction and training of staff.
Service development

To proactively identify, develop and acquire new business opportunities. To proactively develop and streamline current services. To both contribute and encourage team contribution towards service improvement and achieving and maintaining CQC standards. To represent the organisation positively at all times in communication and meetings with professional and external agencies. To address issues of quality, risk assess own performance and take accountability for own actions and those of their team either directly or under supervision. To effectively manage own time, workload and resources.
Administration

To prepare robust business plans for the consideration of the Senior Management Team. These must contain relevant market data, appropriate Key Performance Indicators and be mindful of Patient Safety, Efficiency and Value for Money. To effectively use the IT software provided, check emails daily and keep accurate records. To ensure adherence to lone working procedures as required.
Customer care

To deliver care according to NSF and NICE guidelines. To pro-actively make contact with existing clients on a timely basis to maintain and develop relationships and gather positive feedback and areas of development. To work with current and prospective clients to discuss how the organisation may be of benefit to them. To contribute effectively to delivering a first-class patient focused service. To follow the organisation's complaint reporting procedures always taking ownership and highlighting any errors made.
Miscellaneous

To carry out efficiently all associated tasks to support the smooth running of the service that may be assigned to you by the Senior Management Team. Adhere to Disclosure and Barring Service Checks
