About the role
Are you passionate about enhancing the well-being of citizens and helping individuals achieve stability and independence? We are seeking a dedicated Lead Worker to join our innovativeaccommodation-based health & wellbeing support servicedelivered across Birmingham.
This role focuses on providing person-centred, strength-based support to adults facing multiple challenges, ensuring they access the resources and services they need to thrive. You will work within a holistic and preventative framework that prioritizes community integration, early intervention, and collaboration.
What you'll be doing
Deliver high-quality, personalised support tailored to individuals' unique needs and circumstances.
Facilitate access to community resources, advice, and guidance to empower informed decision-making.
Collaborate with multi-disciplinary teams and referral agencies, ensuring integrated and effective service delivery.
Promote independence and resilience while supporting well-being and safeguarding.
Co-produce solutions with service users to reflect the personal goals of the individual.
Maintain records, track outcomes, and provide reports in line with service objectives.
Who we're looking for?
Have knowledge of, or lived through some of the complex challenges are residents face.
Be empathetic, friendly and a have a can-do attitude.
Demonstrate a strong commitment to equality, fairness, and safeguarding.
Possess excellent interpersonal and communication skills.
Be skilled in multi-agency collaboration and delivering person-centred support.
Have a knowledge of PIE and Trauma informed approaches.
Enjoy problem solving & be committed to continuous improvement.
Be willing & able to work at any of our sites across Birmingham.
