We are seeking dedicated and enthusiastic Social Workers to join our Learning Disabilities team, delivering vital services to adults with learning disabilities and other complex needs.
About the Role:
In this role, you will:
Undertake strength-based social care assessments in line with the Care Act.
Produce outcome-focused support plans tailored to individual needs.
Deliver targeted interventions that safeguard and empower adults with learning disabilities and complex needs.
Promote independence, positive outcomes and the effective use of assistive technology.
Work collaboratively with colleagues across Council departments, NHS Southwark and other key partners.
About You:
You are a qualified and registered Social Worker with Social Work England, with relevant experience in social work practice.
You possess the professional skills and personal attributes to guide families, navigate complex cases and champion the best outcomes for adults.
You are committed to working in a diverse community, applying a strength-based approach that puts individuals and families at the centre of your practice.

sits within Southwark's
All Age Disability pathway
, which delivers integrated social care services from 0-25 and beyond. Our team provides a comprehensive pathway for adults with learning disabilities and eligible social care needs.
