Learning Disability Nurse Acute Liasion

Job Description

The post hold will be joining a small but experienced team, whom provide additional support for client whom have a learning disability who are admitted into Acute hospitals, both Coventry and Rugby.



The job will entail joint working with the acute hospitals alongside the Community Learning Disability team and Community services to ensure that individuals with a learning Disability have equal access to services . To support and guide Acute colleagues in processes such as communication needs, reasonable adjustments, consent and capacity.



Actively promote the role of the liaison nurse within the acute trust, ensuring such information is also accessible to people with a learning disability and their carers.

Identify barriers to accessing acute health care services, for adults with a learning disability, and plan actions and initiatives to overcome and assist easier access to the acute services.



Develop closer links between primary healthcare services and secondary care in order to help facilitate a smooth pathway through acute services.



This is to enable continuity for patients and their families/support workers when accessing both elective and emergency treatment. Planning effective care whilst in hospital, and then assist with comprehensive discharge/aftercare arrangements for adult patients with a learning disability.



Provide health promotion/raising awareness workshops and educational sessions to groups and individuals, both within the acute and community settings.



Identify an action plan to meet the additional health needs of people with a learning disability who come from black and minority ethnic background.



Work with the Community Learning Disability Nurses to ensure effective health facilitation and a smoother pathway from primary care into secondary care.



Complete nursing assessments as appropriate in a variety of formats including continuing healthcare framework.
