Leeds - Learning Disabilities Team - \xc2\xa330.98PH (Umbrella) - We are looking for enthusiastic and motivated Senior Social Workers to join our well established teams in Leeds Adult Social Care - Learning Disabilities Team

This role is to support Adult Social worker. The team is made up of staff from health and social care, working together to provide a holistic assessment and support plan to ensure outcomes are achieved and independence is maintained wherever possible

To deliver assessment and care management services to adults considered to require community care and support to maximise and maintain their independence and dignity.

RESPONSIBILITIES
  • Prepare for, and work with individuals, families, carers, groups, and communities to assess their needs and circumstances
  • Plan, carry out, review, and evaluate social work practice, with individuals, families, carers, groups, communities, and other professionals:
  • Support individuals to represent their needs, views, and circumstances:
  • Manage and be accountable, with supervision and support, for your own social work practice within your organisation:
  • Demonstrate professional competence in social work practice.
