This opportunity is a chance to not only positively change the lives of families and children but also to gain valuable experience and help develop your career. The children or young person will have a severe and enduring learning disability and may include physical disability.
This is not a hands on health role providing personal care for the young person but is about enabling the family. You will complete an assessment with each family on your case load and develop an Early Help plan that identifies the support needed. This may include helping get access to the right financial support, mental health support or support in the community.
You will be working with a committed and supporting management team that will enable you to do your job to the best of your ability.
