Team Manager - LIFE Service (Discharge to Assess Pathway)

We are seeking a dynamic and experienced Team Manager to lead our LIFE Service - Discharge to Assess (D2A) pathway. This is a pivotal role focused on prevention, early intervention, and whole-family support, helping Croydon residents live independent, fulfilling lives.



Key Responsibilities




Lead and manage a multidisciplinary team of Social Workers, Therapists and Health & Wellbeing Assessors. Oversee the operational delivery of the D2A pathway, ensuring high-quality, person-centred care. Use tools like the Community Led Support resource wheel to support strengths-based conversations and planning. Foster strong partnerships across health, social care, and the voluntary sector. Ensure compliance with statutory guidance and contribute to the transformation of health and social care services. Promote a culture of continuous improvement, innovation, and positive outcomes for residents.

You will be a confident and compassionate leader with:


A recognised social work qualification and current registration with Social Work England. Proven experience in adult social care, ideally within hospital discharge or reablement services. Strong understanding of strengths-based and community-led support models. Excellent communication and partnership-building skills. A commitment to embedding cultural and behavioural change that puts residents at the heart of everything we do.
