Job Description

This is a 10PA post (8DCC, 2 SPA) based at Charing Cross Hospital Campus of Imperial College Healthcare NHS Trust.



Medicine for the Elderly (MFE) services have evolved over recent years at Charing Cross Hospital (CXH), so there is now a well-functioning Acute Frailty Unit, Older Persons Rapid Access Clinic (OPRAC) and ED liaison service in addition to the 48 acute MFE beds (8S and 8W), 15 rehab beds, Older Persons Assessment and Liaison Service (OPAL), outpatients and community services. With the expanding services, the Consultant team needs an additional Geriatrician to join the unit in running these services day-to-day, to ensure there is always Consultant input.



The key areas related to this role are the acute Elderly Medicine wards - 8 West Ward (23 beds), 8 South (25 beds), 5 West (15 beds) and the 'Interface' service (Acute Frailty Unit, OPRAC and ED Liaison). This post holder will work with three other colleagues rotating across these areas as well as doing acute medical on-calls.



Final agreement on the timetable of commitments will be agreed on taking up the post and then reviewed at least annually as part of the job planning.



DCC:

8 PA's per week (including clinical activity, clinically related activity, predictable and unpredictable emergency work)



SPA:

2 PA's per week (including audit, clinical governance, CPD, teaching, research, educational supervision of junior staff). The Consultant is expected to initiate and supervise audits performed by juniors; audits are presented at the regular site-specific departmental meetings. There are also regular morbidity and mortality meetings on each site, the results of which are fed back to the Departmental Quality and Safety Lead and Audit Lead and reviewed at quarterly Trustwide departmental meetings.

The postholder will rotate between 3 clinical areas, joining 3 other Consultant Geriatricians. The areas are two wards, 8W and 8S, a rehab ward, 5W, and Acute Frailty Services (Older Persons Rapid Access Clinic - OPRAC, and Emergency Department liaison). The team also covers a weekly outpatient clinic. At any time the fourth Consultant will be the 'cover Consultant', covering leave or on-calls - this means that all clinical areas will have dedicated Consultant input. The rotations will be 2-3 months in each area (8W, 8S, OPRAC/5W and cover).



8 West & 8 South



The postholder will lead a team of junior doctors caring for acutely ill older patients on each of these wards at Charing Cross Hospital. This involves routine Mon-Fri input: twice weekly full ward rounds, daily board rounds and reviews of new patients, weekly MDT meetings and family meetings. The postholder will provide leadership to the multidisciplinary team and work closely with the Ward Manager and colleagues in other areas of the Elderly Medicine Department within the hospital and community.



OPRAC / 5 West ("Lady Skinner" ward)



The postholder will also review up to 3 patients a day in OPRAC. This service runs 5 days a week, Mon-Fri. 5 West ward is a step-down rehabilitation ward although the patients often have need for ongoing medical expertise at a lower intensity than the acute wards.



Cover Period



During the 'cover' period, the post holder will cover other clinical areas for 1-2 weeks at a time when colleagues are on leave or on call.



Community Services



The postholder will contribute to our community geriatrics provision, with consultant time commissioned for various services in the West London area including input into the "My Care My Way" community hub and nursing home reviews.
