Located on the Isle of Wight, 8 minutes from St Marys Hospital and 26 minutes away from Cowes Ferry Terminal.In this role you'll be working with a multi-disciplinary team that will support you and help you succeed.This position is full time, youll receive an annual salary up to xc2xa352,507 per annum - depending on experience. Hours:8 hour shift patterns8am-4pm 9am-5pmHow will we support you?Bespoke induction, including our Introducing Health in Justice training courseCompetency frameworkRegional and national career development opportunitiesOur bespoke Learning Management System to address your learning needsSupport from the wider teamCandidates are required to have a Legal right to work in the UK and due to the nature of the role, offers of employment will be subject to additional vetting and security checks.Main duties of the jobAs a Long Term Conditions Nurse, your responsibilities will vary. You will

• Be responsible for ensuring a comprehensive handover of services when patients are transferring across wings or custodial care settings.
• You will identify immediate health needs including, any external services or prescribing requirements that prisoners may have received e.g. prior to remand or repatriation onto a wing
• A key part of this role will include the facilitation of arrangements between agencies involved in delivering a complex care package to ensure effective collaboration and stability for the patient in delivery of that care. This requires close joint working with other in-house healthcare services and departments within the prison such as Pharmacy, Education, Offender Management, Substance Misuse and Physical Education, etc.
• Provide proactive intervention to urgently address improvement recommendations from CQC and HMPP inspections.

• Registered Nurse- General with relevant NMC Registration
• Educated to degree level
• Knowledge of health and social care models across the criminal justice system and of national and local initiatives relating to physical health
