We have an exciting opportunity for a Long Term Conditions Nurse to join our Primary Care team at HMP Full Sutton, a

men's Prison located in

Full Sutton, York.





This site is located in a rural location with their own parking on site, an on-site gym, cafe, team building days and well-being events annually, regular 1-1's and good scope for development, vaccination training, additional advanced qualifications.


Hours





30 hours a week - Working 4 days a week, hours are negotiable and no weekends. 8.30-9am start ideally, finishing around 4.30-5pm.


Salary





This position is full time, you'll receive an annual salary up to 49,175 per annum FTE - depending on experience.

What you'll be doing





As a Long Term Conditions Nurse, you will:


Ensure a comprehensive handover of services when patients are transferring across wings or custodial care settings. Identify immediate health needs including, any external services or prescribing requirements that prisoners
may have received e.g. prior to remand or repatriation onto a wing
A key part of this role will include the facilitation of arrangements between agencies involved in delivering a complex care package to ensure effective collaboration and stability for the patient in delivery of that care. This requires joint working with in-house healthcare services and departments within the prison such as Pharmacy, Education, Offender Management, Substance Misuse and Physical Education, etc. Provide proactive intervention to urgently address improvement recommendations from CQC and HMPP inspections Deliver continuity of care across health and social care agencies from courts, to custody and community Inspire the continuous improvement of MDT working across HMP and healthcare teams.

Qualifications required




Registered Nurse- General with relevant NMC Registration Experience in long term conditions Advanced Nurse Prescriber/ Practitioner - Desirable Educated to degree level Knowledge of health and social care models across the criminal justice system and of national and local initiatives relating to physical health Knowledge of Evidence Based Practice and Clinical Governance Experience of assessing the needs of and working with individuals with complex health and social care needs.

Although not essential ideal applicants would have experience in a hospital, community or prison environment.
