Medical Records Summariser
to join our team.
Role Summary:
The postholder will be responsible for accurately summarising and coding incoming patient medical records into our clinical system. This is a key role in ensuring that patient records are complete, accurate and accessible, supporting safe clinical decision-making.
Key Responsibilities:
Read and interpret patient medical records (paper and electronic).
Summarise relevant medical history into the clinical system in line with practice protocols.
Code diagnoses, significant past medical history, operations, and key medications using appropriate clinical coding.
Ensure data quality, accuracy and confidentiality at all times.
Liaise with clinicians and administrative staff to clarify information where needed.
Contribute to maintaining up-to-date and accurate patient records.
Essential Skills & Experience:
Excellent attention to detail and accuracy.
Good IT skills with experience of using clinical systems.
Understanding of medical terminology.
Ability to work independently and as part of a team.
Commitment to patient confidentiality and data protection.
Desirable:
Previous experience in medical records summarising/coding.
Experience in a primary care or healthcare setting.
Knowledge of read/SNOMED coding.
