Job Description

To work as part of the Medicines Management Domiciliary Care Team, helping to facilitate medicine administration from original packs by Domiciliary Care Workers to patients receiving a package of care in their own home.

There is one full time post available to cover Bradford.

Deliver training and competency programmes in medicines administration to care workers and stakeholders.

Provide support to care providers in medicines management including domiciliary home visits and level of support assessments.

Assist in the day-to-day running of domiciliary care medicines management services including administrative tasks and dealing with queries from Care Providers, Social Workers and Health Care Professionals.

Aid in the planning, development and expansion of domiciliary care medicines management services in new areas, to improve medicines management, promote independence and improve care outcomes for people requiring support in their own homes.

Work proactively with others as part of a multi-disciplinary approach to support them in the coordination, planning and effective discharge process, including resolving medicines management and compliance aid queries.

Auditing all VCS current service users medication.

Liaise with community pharmacies and other stakeholders regarding the provision of medicines administration charts, compliance aids, medication reviews etc.

Link with district nurses, GP practices, specialist clinics etc to ensure appropriate information and systems are in place to enable safe administration of particular medicines.

Must have two years minimum experience of medication management

Will also be part of on-call service providing 24hr support to staff and service users 1 out of every 4 weeks

,, most of our office team started out as carers and have now progressed their careers. Our friendly office team are here to provide support 24 hours a day. We have 2 out of hour phone lines that provide support to all our carers when the office is closed
